Hindernisse zwischen Europäischen Staaten bei der Datensammlung und dem Datenaustausch bei Forschung an seltenen kindlichen Erkrankungen- Systematische Literaturanalyse und Patientenperspektive
Kinder werden viel zu häufig als „Waisen der Medizin“ gesehen. Der Grund hierfür liegt darin, dass Forschung an pädiatrischen selten Erkrankungen und die Fortentwicklung deren Therapieoptionen unter anderem auf Grund der internationalen Diskrepanz der Gesetze und der Vulnerabilität der Kinder, fehlende einheitliche Sprache und uneinheitlicher Bewertung der Ethikanträge dramatisch erschwert wird. Da signifikante Ergebnisse allerdings nur bei multizentrischer Forschung an seltenen Erkrankungen zu erwarten sind, ist eine internationale Kollaboration der Forschung dringend nötig und muss gefördert werden.
Aktuell gibt es bisher noch kein EU-weit geltendes einheitliches Regelwerk für die Forschung, die Lagerung und den Austausch von Proben, an dem sich Forscher orientieren können. Da viele Forscher durch die ethischen Bestimmungen an der Forschung behindert werden und Kinder jedoch ein Recht auf ein Fortschreiten der Medizin und der Therapieoptionen auch bei seltenen Erkrankungen haben, ist eine Vereinheitlichung der Ethikkommissionen hinsichtlich Forschung an seltenen Erkrankungen unumgänglich.
Diese Arbeit wurde von der Europäischen Union gefördert, um den aktuellen Stand der Situation zu beleuchten und Empfehlungen für ein Regelwerk zu erarbeiten, damit Forschung in Zukunft auch multizentrisch inklusiv der Lagerung und der Wiederverwendung von Proben möglich werden kann. Hierfür wurden eine systematische Literaturanalyse und mehrere Fragebögen entwickelt. In einem anderen Teil der Arbeit wurden vor allem auf die Prozess-Hindernisse und die optimale Vorgehensweise bei der Antragsstellung eingegangen. Dieser Teil der Arbeit dokumentiert die Literaturanalyse und rundet die Empfehlungen mit der Patientenbefragung ab.
In der Literatur zeigte sich eine sehr große Bandbreite vorhandener Literatur, meist aber von niedrigem Evidenzlevel, Studien gab es zu diesem Thema nur selten, auch wenig systematische Übersichtsarbeiten. Die Arbeit zeigte sich sehr herausfordernd, unter anderem da eine Einengung des Interessenbereiches nur schwer möglich war, aber auch, da sich regionale Unterschiede bezüglich der geltenden Gesetze und Verordnungen herausstellten. Ein EU-weites Regelwerk, das die Lagerung, Wiederverwendung und den Austausch von kindlichen Proben regelt, ist bisher nicht definiert.
Nachdem die Ergebnisse des Test-Antrags (Morbus Behçet), der Telefoninterviews und der Befragung der klinischen Forscher mit den Ergebnissen der Literatur verglichen, diskutiert und ausgewertet wurden, entstanden im Rahmen der Arbeitsgruppe SHARE WP7 die Empfehlungen, um die identifizierten Hürden in Zukunft zu bewältigen. Diese wurden primär in Form eines Patientenfragebogens überprüft, um somit auch insbesondere auf die Vorstellungen, Wünsche und Ideen der Patienten einzugehen. Hierbei zeigte sich vorwiegend eine deutliche Zustimmung und ein zufriedenes Annehmen der Empfehlungen.
In der Zusammenschau der Ergebnisse aus beiden Teilen der Doktorarbeit wurden Hindernisse identifiziert und Empfehlungen, diese zu bewältigen ausgearbeitet. Anschließend wurden die Empfehlungen mit ethischen Experten überarbeitet und durch einen Juristen auf Legalität im europäischen Rahmen überprüft.
Letztendlich wurden ĂĽber die Empfehlungen im Rahmen der SHARE Mitglieder diskutiert und abgestimmt. Die endgĂĽltigen Empfehlungen setzen sich aus folgenden Themengebieten zusammen:
• Grundlegendes Prinzip (Leitgedanke, Leitmotiv) (drei Empfehlungen)
• Empfehlungen für die Ethikkommission (vier Empfehlungen)
• Pädiatrische Prinzipien (zwei Empfehlungen)
• Einverständnis zu pädiatrischer Forschung (fünf Empfehlungen)
• Pädiatrische Daten- und Biobanken (zwei Empfehlungen)
• Austausch von Daten und Proben (drei Empfehlungen)
• Kommerzialisierung und Zugriff dritter Parteien (zwei Empfehlungen)
Da bisher keine EU-weite Empfehlungen fĂĽr multizentrische Forschungsprojekte mit internationalen Daten- und Probenaustausch existieren, sind dies die ersten ihrer Art. Ein einheitliches Regelwerk bezĂĽglich legaler und ethischer Aspekte ist unabdingbar, ebenso wie eine evidenzbasierte Implementierung dieser.
Nur so kann vermieden werden, dass Kinder, insbesondere bei seltenen Erkrankungen, weiterhin als „Waisen der Medizin“ behandelt werden. Die Tür in eine Zukunft mit neuen, lebensverändernden Entdeckungen und neuartigen therapeutische Optionen kann geöffnet werden
A percepção de gestores e gestoras sobre a influĂŞncia do gĂŞnero no exercĂcio de cargos de liderança: um estudo de caso em uma empresa prestadora de serviços.

A pergunta de pesquisa Ă©: “Qual a percepção de lĂderes acerca da influĂŞncia do gĂŞnero sobre o desempenho do papel de liderança?” O objetivo geral do estudo Ă©: “Analisar a percepção de lĂderes acerca da influĂŞncia do gĂŞnero sobre o desempenho do papel de liderança”. Foram realizadas entrevistas com seis gestores da Liderança Serviços, empresa de serviços localizada em SĂŁo JosĂ©-SC. O estudo tem sua abordagem qualitativa, já que nĂŁo há instrumentos estatĂsticos na análise de dados. O procedimento adotado para a estratĂ©gia de pesquisa se enquadra no estudo de caso, permitindo compreender com maior profundidade a realidade da empresa. As entrevistas forneceram base para uma análise aprofundada dos dados e para uma discussĂŁo sobre formatos de gestĂŁo, atributos de gĂŞnero, desigualdades sociais, famĂlia, etc. Com a pesquisa foi possĂvel concluir que há influĂŞncia do gĂŞnero nos formatos de gestĂŁo, contudo, fatores como: faixa etária, valores, experiĂŞncia, afinidade com a equipe, personalidade, tambĂ©m interferem de alguma forma.In this case study it is discussed the influence of the genders male and female at the leadership models. The research question is: “What is the perception of leaders about the influence of gender on the performance of the leading role?” The general objective of the study is: “Analyze the perception of leaders about the influence of gender on the performance of leading role”. It were conducted six intreviews with managers from Liderança Serviços, a service company localized in SĂŁo JosĂ©-SC. The study has a qualitative approach, because there are no statistical variables. The procedure adopted for the research strategy is the study of case, which allows to understand deeply the reality of the organization. The intreviews provided base for a data analysis and for a discussion about the management formats, gender attributes, social inequality, family, etc. With the research it was possible to conclude that there is influence of the gender at the management format, however, factors such as: age group, values, experience, rapport with the staff, personality, also interfere some way
A large-scale and PCR-referenced vocal audio dataset for COVID-19
The UK COVID-19 Vocal Audio Dataset is designed for the training and
evaluation of machine learning models that classify SARS-CoV-2 infection status
or associated respiratory symptoms using vocal audio. The UK Health Security
Agency recruited voluntary participants through the national Test and Trace
programme and the REACT-1 survey in England from March 2021 to March 2022,
during dominant transmission of the Alpha and Delta SARS-CoV-2 variants and
some Omicron variant sublineages. Audio recordings of volitional coughs,
exhalations, and speech were collected in the 'Speak up to help beat
coronavirus' digital survey alongside demographic, self-reported symptom and
respiratory condition data, and linked to SARS-CoV-2 test results. The UK
COVID-19 Vocal Audio Dataset represents the largest collection of SARS-CoV-2
PCR-referenced audio recordings to date. PCR results were linked to 70,794 of
72,999 participants and 24,155 of 25,776 positive cases. Respiratory symptoms
were reported by 45.62% of participants. This dataset has additional potential
uses for bioacoustics research, with 11.30% participants reporting asthma, and
27.20% with linked influenza PCR test results.Comment: 37 pages, 4 figure
Recommendations for collaborative paediatric research including biobanking in Europe: a Single Hub and Access point for paediatric Rheumatology in Europe (SHARE) initiative
Innovative research in childhood rheumatic diseases mandates international collaborations. However, researchers struggle with significant regulatory heterogeneity; an enabling European Union (EU)-wide framework is missing. The aims of the study were to systematically review the evidence for best practice and to establish recommendations for collaborative research. The Paediatric Rheumatology European Single Hub and Access point for paediatric Rheumatology in Europe (SHARE) project enabled a scoping review and expert discussion, which then informed the systematic literature review. Published evidence was synthesised; recommendations were drafted. An iterative review process and consultations with Ethics Committees and European experts for ethical and legal aspects of paediatric research refined the recommendations. SHARE experts and patient representatives vetted the proposed recommendations at a consensus meeting using Nominal Group Technique. Agreement of 80% was mandatory for inclusion. The systematic literature review returned 1319 records. A total of 223 full-text publications plus 22 international normative documents were reviewed; 85 publications and 16 normative documents were included. A total of 21 recommendations were established including general principles (1-3), ethics (4-7), paediatric principles (8 and 9), consent to paediatric research (10-14), paediatric databank and biobank (15 and 16), sharing of data and samples (17-19), and commercialisation and third parties (20 and 21). The refined recommendations resulted in an agreement of >80% for all recommendations. The SHARE initiative established the first recommendations for Paediatric Rheumatology collaborative research across borders in Europe. These provide strong support for an urgently needed European framework and evidence-based guidance for its implementation. Such changes will promote research in children with rheumatic diseases
Serological detection of anti-leptospiral antibodies in outdoor cats in Switzerland
Leptospirosis is endemic in Switzerland affecting a broad range of hosts. The aim of this study was to estimate the exposure of cats to Leptospira in Switzerland.
Plasma samples from 107 outdoor cats with an array of clinical problems were tested via microscopic agglutination test for the presence of anti-leptospiral antibodies against 12 serovars of 9 serogroups. Using a reciprocal cut-off titre of 1:100, an overall seroprevalence of 10.3% (95%CI 5.2–17.7) was observed. Seroreactivity against serovars Bratislava (n = 6), Australis (2), Pomona (3) and Copenhageni (1) was detected with reciprocal titres ranging from 1:100 to 1:800. The serologic status of the cats was independent of the disease group (p = 0.62). These results show that cats in Switzerland are commonly exposed to Leptospira and confirm the importance of serogroup Australis in this region
Canine leptospirosis in Switzerland—A prospective cross-sectional study examining seroprevalence, risk factors and urinary shedding of pathogenic leptospires
Leptospirosis is an important worldwide zoonosis. While human leptospirosis remains rare in Switzerland, the incidence of canine leptospirosis is unusually high compared to other European countries. The aims of this cross-sectional study were to determine the exposure of asymtomatic dogs to pathogenic Leptospira in Switzerland, to characterise risk factors associated with seropositivity and to determine the prevalence of urinary shedding. Sampling was stratified to cover the whole of Switzerland. Sera were tested by microscopic agglutination test for antibodies against a panel of 12 serovars. Urine was tested for pathogenic Leptospira using a LipL32 real-time PCR. Of 377 sera, 55.7% (95%CI 51.2-60.7) showed a reciprocal MAT titre of ≥1:40 and 24.9% (95%CI 20.7-29.4) of ≥1:100 to at least one serovar. Seropositivity (MAT ≥1:100) was most common to serovars Australis (14.9%; 95% CI 11.4-18.6) and Bratislava (8.8%; 95%CI 6.1-11.7), followed by Copenhageni (6.1%; 95%CI 3.7-8.5), Canicola (5%; 95%CI 2.9-7.4), Grippotyphosa (4.5%; 95%CI 2.7-6.9), Pomona (4%; 95%CI 2.1-6.1), Autumnalis (2.7%; 95%CI 1.3-4.2) and Icterohaemorrhagiae (1.6%; 95%CI 0.5-2.9). In unvaccinated dogs (n = 84) the prevalence of a MAT titre ≥100 was 17.9% (95%CI 10.7-26.2), with a similar distribution of reactive serovars. Variables associated with seropositivity (≥1:40) to any serovar included age (OR 1.29/year; 95%CI: 1.1-1.5) and bioregion with higher risks in the regions Northern Alps (OR 14.5; 95%CI 2.2-292.7), Central Plateau (OR 12.3; 95%CI 2.0-244.1) and Jura (OR 11.2; 95%CI 1.7-226.7) compared to Southern Central Alps. Dogs living with horses were significantly more likely to have antibodies to serovar Bratislava (OR 4.68;95%CI 1.2-17.2). Hunting was a significant risk factor for seropositivtiy to serovar Grippotyphosa (OR 8.03; 95%CI 1.6-30.8). Urine qPCR positivity was uncommon (1/408 dogs; 0.2%; 95% CI0-0.7). These results demonstrate that dogs in Switzerland are commonly exposed to pathogenic Leptospira; however, the risk of dogs contributing to the spread of Leptospira in the environment appears low
Écritures et pratiques de l’amitié dans l’Italie médiévale
Le présent ouvrage s’adresse avant tout à un public de médiévistes, car il répond à un besoin concernant l’approche de ce thème dans la culture italienne. En effet, on observe un manque dans ce domaine. Pourtant, des travaux récents ont été publiés sur l’amitié, mais ils sont soit très généraux, soit centrés très spécifiquement sur un auteur (par exemple sur saint Augustin en 2008 et en 2009), ou sur un texte particulier (par exemple sur l'Ethique à Nicomaque (Brépols, 2007) ; et ne concernent pas la culture italienne. L’ouvrage s’adresse aussi à des lecteurs non médiévistes s’intéressant aux époques postérieures, car il fournit des instruments de compréhension des sources de la réflexion moderne sur le thème de l’amitié, réflexion qui s’est ensuite diffusée dans les autres pays d’Occident
